OKINAWA, Japan (January 7, 2019)— Behavioral health specialists report depression and suicide ideation rates increase during the holiday season and into the post-holiday period in the Marine Corps, according to the Headquarters Marine Corps Force Preservation Directorate. Marines may feel lonelier during these times as a result of being away from their families and supporters, said Shannon Hutchinson, the 3rd Marine Division behavioral health specialist. Marines then question their belonging and wonder if people would care if they weren’t around.
One of the 14 Marine Corps leadership principles states, “know your Marines and look out for their welfare.” Hutchinson said leaders should make it a regular practice to talk about taking care of yourselves and each other when it comes to managing stress and everyday issues. She also advised that regular counseling sessions should include discussions about issues other than work, like family, finance and mental health. In order to be “ready to fight tonight” and achieve the Marine Corps mission, Marines must be mentally and spiritually ready, said Hutchinson.
“The truth is it’s your responsibility and part of the commitment you made to the Marine Corps to be ready to handle whatever the Marine Corps needs of you at any time,” said Hutchinson. “We perform corrective and preventative maintenance on equipment so why wouldn’t we do that for ourselves and our fellow Marines?”

As military members, we’re required to attend suicide prevention training to recognize the signs amongst peers and lead them to the appropriate resources, but even then it’s difficult to know if someone is actually contemplating suicide unless we truly know that person. According to Commander Salee Oboza, the operational stress control and readiness training clinical provider, the best way to recognize the signs is to get to know your peers and care about their well-being.
“People don't hesitate to help when someone is limping, but people tend to hesitate and turn the other way when someone is having a difficult time with their emotions,” said Oboza. “Emotional pain is the same as physical pain, it needs treatment for it to be better.”
To defeat the stigma surrounding utilizing mental health resources, Oboza advises Marines to take every suicidal comment seriously and never make fun of the topic. She also encourages commands to invite the providers into work spaces to introduce themselves and their capabilities. Some resources found on base are the Behavioral Health Community Counseling Program and Family Advocacy Program, military and family life counselors, the Families Overcoming Under Stress team, the operational stress control and readiness team, the U.S. Naval Hospital mental health department, the substance abuse control officer and the command chaplain.
“Asking for help will not make you appear weak and/or less of a Marine,” said Hutchinson. “You have a role in enhancing overall readiness. Protect yourself and others against behavioral health issues and take action to prevent incidents.”

Title Seeing Titan with infrared eyes

Saturn’s moon Titan is enveloped in a thick atmosphere, but through the infrared eyes of the international Cassini mission, the moon’s myriad surface features are revealed in this exquisite global mosaic.

Observing the surface of Saturn’s largest moon, Titan, in visible light is difficult due to the globe-enshrouding haze that envelops the moon. On 14 January 2005, the mystery as to what lay beneath the thick atmosphere was revealed as ESA’s Huygens probe – carried to Titan by Cassini – made the first successful landing on a world in the outer Solar System. During the two-and-a-half hour descent under parachute, features that looked remarkably like shore lines and river systems on Earth appeared from the haze. But rather than water, with surface temperatures of around –180ºC, the fluid involved here is methane, a simple organic compound that also contributes to the moon’s obscuring atmosphere.

Thanks to Cassini, which studied Saturn and its rings and moons for thirteen years, Titan was extensively mapped and analysed. One result is this stunning sequence of images created using data acquired by Cassini’s Visual and Infrared Mapping Spectrometer (VIMS), whose infrared observations peered through Titan’s atmosphere, complementing the views obtained by Huygens during descent and on the surface. The maps combine data from the multitude of different observations made under a wide variety of illumination and viewing conditions over the course of the mission, stitched together in a seamless mosaic to provide the best representation of Titan’s surface to date.

The colours reflect variations in materials on the moon’s surface. For example, the moon’s equatorial dune fields appear a consistent brown colour, while bluish and purple hues may indicate materials enriched in water ice.

Nous avons une bonne nouvelle à vous annoncer en ce début 2019 : des militants écologistes indonésiens ont reçu le très renommé prix Yap Thiam Hien pour les droits de l’Homme. Le jury a ainsi récompensé des personnes agissant à la préservation de la nature, et ce à une époque où de plus en plus de défenseurs de l’environnement sont menacés de persécutions.

Le prix a été décerné conjointement à la communauté de Sedulur Sikep sur l’île de Java et à la défenseuse de l’environnement Eva Bande originaire des Célèbes (ou Sulawesi).

La communauté pacifiste de Sedulur Sikep s’oppose à la destruction de la chaine montagneuse karstique des Kendeng pour la production de ciment, notamment par Indocement, une filiale de l’entreprise allemande HeidelbergCement. Leur action non violente pour la défense de notre Terre-mère est exemplaire.

Aux Célèbes, Eva Bande lutte aux côtés d’indigènes et d’agriculteurs contre l’accaparement de terres, la violence et la déforestation par des compagnies minières et des entreprises d’huile de palme. Elle est notamment soutenue par notre organisation partenaire Jatam. Un hommage est ainsi rendu aux nombreux écologistes poursuivis en justice et criminalisés.

“Zero polio transmission and health for all”, WHO Director-General gives new year’s wish to the people of Afghanistan and Pakistan

WHO Director-General Dr Tedros Adhanom Ghebreyesus highlighted WHO’s commitment to the final push to eradicate polio on a 4-day visit to Afghanistan and Pakistan - the only two countries where wild poliovirus cases were reported last year. He also commended the governments of both countries for their efforts to provide universal access to health services.

Dr Tedros, together with WHO Regional Director for the Eastern Mediterranean Dr Ahmed Al-Mandhari, met with heads of state and senior government officials in both countries and witnessed first-hand WHO-supported health programmes.

He also visited the Emergency Operations Centre for Polio Eradication in Islamabad, Pakistan, where he commended the work of government and partners as “one team under one roof” and highlighted the critical importance of working closely with Afghanistan to prevent cross-border transmission.

“We must all give our best on this last mile to eradicate polio once and for all. My wish for 2019 is for zero polio transmission. You have WHO’s full support to help reach every child and stop this virus for good,” Dr Tedros said.

On his visit to Afghanistan on 5-6 January, Dr Tedros met with HE President Dr Ashraf Ghani, HE Chief Executive Dr Abdullah Abdullah, the Council of Ministers, representatives of key partners and nongovernmental organizations active in health.

Together with HE Minister of Public Health Dr Ferozuddin Feroz, he launched the newly developed Integrated Package of Essential Health Services. This package includes the most cost-effective evidence-based interventions that reflect the most common causes of mortality and morbidity in the country. It keeps the focus on primary health care but also adds noncommunicable diseases and trauma care. Dr Tedros confirmed WHO’s support to the government to develop financing options to help ensure access to health services for all Afghans.

Dr Tedros also visited the WHO-supported Trauma Care Hospital run by the Italian NGO Emergency in Kabul, where Dr Tedros thanked humanitarian workers for their important work. He praised the close collaboration between WHO, Ministry of Health and International NGOs like Emergency, so that provision of essential trauma care can be accessed by people who need it most.

In Pakistan on 7-8 January, Dr Tedros met with Prime Minister Imran Khan and several senior officials including Federal Minister for National Health Regulations & Coordination Mr Aamer, Mehmood Kiani, Minister of Foreign Affairs and Federal Minister for Human Rights Dr Shireen Mazari.

Dr Tedros accompanied the President of Pakistan, Arif Alvi, to the launch of the first Pakistan Nursing and Midwifery Summit and the Nursing Now campaign. Pakistan faces a critical shortage of health workers including nurses and midwives. The country needs more than 720 000 nurses to achieve universal health coverage by 2030.

He also visited a basic health centre in Shah Allah Ditta where WHO signed an agreement with the Government of Pakistan to develop a model health care system for universal health coverage in Islamabad. Dr Tedros commended the Government for its initiatives to tax tobacco and sugary drinks, as well as its plans to increase the health budget to 5% of GDP by 2023 (from the current 0.9% of GDP).

The visit took place shortly after WHO Director-General Dr Tedros Adhanom Ghebreyesus took over the Chair of the Polio Oversight Board, which guides and oversees the Global Polio Eradication Initiative – spearheaded by national governments, WHO, Rotary International, UNICEF, CDC and the Bill & Melinda Gates Foundation - as a clear sign that the eradication of this disease is a priority for WHO.

As recently as 30 years ago, wild poliovirus paralysed more than 350 000 children in more than 125 countries every year. In 2018 there were fewer than 30 reported cases in just two countries – Afghanistan and Pakistan.

Polio eradication requires high immunization coverage everywhere, worldwide, to block transmission of this extremely contagious virus. Unfortunately, children are still missing out on vaccination for various reasons including lack of infrastructure, remote locations, population movement, conflict and insecurity and resistance to vaccination.

Failure to eradicate polio from these last remaining strongholds could result in a resurgence of the disease, with as many as 200 000 new cases predicted worldwide every year within 10 years.

Evolution of a black hole
The cover image shows an artist’s impression of a black hole that has started to accrete material from a nearby star. The properties of accretion flow for a stellar-mass black hole can change on a timescale of days to months. When the black hole ‘turns on’ after accreting material it has a hard (high-energy) X-ray spectrum that is produced by the hot corona (shown in blue) above its accretion disk (yellow). It then changes to a soft (lower-energy) spectrum that is dominated by emissions from the accretion disk. But whether it is a change in disk’s radius or a reduction in the corona that drives this transition has been an open question. In this week’s issue, Erin Kara and her colleagues provide results that may help to settle the debate. The researchers studied a newly identified black hole transient and followed its evolution using instruments on the International Space Station. They found over the course of the hole’s evolution from hard to soft emissions, the corona contracts but the inner edge of the accretion disk stays in the same place.

Adopting a new electronic health record, also known as an EHR, across a large, geographically diverse enterprise is no easy task. To meet the change management needs for this critical effort, the Defense Health Agency has established the Office of the Chief Health Informatics Officer. OCHIO will collaborate with a community of health care clinicians, health information technology experts, and testers to help users of this health information management system seamlessly transition to new technologies and processes.

MHS GENESIS is the single, integrated medical and dental EHR. Scheduled to be systematically fielded at all military hospitals and clinics by 2024, initial testing phases of MHS GENESIS are now complete. The Military Health System is addressing challenges and issues as it prepares for the first wave of deployment next fall, said Air Force Maj. Gen. Lee E. Payne, who is dual-hatted as the Defense Health Agency Assistant Director for Combat Support and MHS EHR Functional Champion.

“I have to set the stage for the frontline users about what they can expect and that’s part of my responsibility,” said Payne. “We’ve been building the capability of the Office of Chief Health Informatics Officer to help us manage the record more in real time and deal with these problems, [and] help build the content configuration necessary to improve the record.”

As the MHS EHR Functional Champion, Payne represents the voice of front-line users and will make critical decisions throughout its deployment. His responsibilities include ensuring solutions are seen from an enterprise perspective and evaluating potential process enhancements. He also serves as the DHA liaison to the Program Executive Office, Defense Healthcare Management Systems, which is responsible for the deployment of MHS GENESIS and management of the rollout plan.

“I want to give you a transparent review of where we are with MHS GENESIS,” said Payne, speaking at the annual meeting of the Society of Federal Health Professionals, better known as AMSUS, in November. “There are some good things happening and I want to be able to share these with you.”

MHS GENESIS launched at four initial operational capability sites in 2017: Fairchild Air Force Base, Naval Health Clinic Oak Harbor, Naval Hospital Bremerton, and Madigan Army Medical Center. Payne said lessons learned from the Pacific Northwest deployment focused on change management, workflows, issue resolution, and training methods.

“We had a change management strategy,” said Payne, adding that they underestimated the magnitude of the change. “I think in some of our minds, at least in my mind, it was, ‘OK, we’ll buy the commercial, off-the-shelf system, you just plug it in, you go to work and you do your job after you get training.’ The system comes about 80-percent configured already, but the MHS is continuing to refine workflows for the new system.

Another critical component of the rollout strategy is helping users understand their workflows, Payne said. “If you try to take your existing business processes, whether that’s in AHLTA or Essentris, and you try to jam that into the new electronic health record, you will fail,” said Payne, referring to legacy EHR systems. “You must adapt your business practices to the new record.”

Payne said MHS GENESIS is more than a system for documenting health care, as AHLTA does. It serves as a care coordination tool and provides standardized workflows that aren’t available in the current systems, a feature that has exposed inconsistencies across the MHS, he said. “Not only do we have variability between the services, but we have variability between our hospitals, and we have variability inside our hospitals and clinics,” said Payne. Communicating workflows and bridging knowledge gaps from old to new processes early in the transition was another important lesson, he continued. “We have to make decisions from an enterprise perspective, not from a local perspective.”

The MHS is also focusing on improving its training approach and content so it ties more closely to workflows. This will include retraining users at the initial sites when training methods change, he said. Peer expert training, where physicians and nurses get trained by experts in their specialty, has been the most effective method.

“We want to get those peer experts trained and experienced with the record, so as we go live, there’s somebody at your elbow to be able to coach you through that process,” said Payne. Having one peer expert per 10 physicians has shown to be a good ratio, he said, a ratio he aims to require for wave-one sites. These include Naval Air Station Lemoore, Travis Air Force Base, and Army Medical Health Clinic Presidio of Monterey, all in California, as well as Mountain Home Air Force Base in southwestern Idaho.

Payne said that as challenges at the IOC sites became evident, OCHIO prioritized the issues and incidents, addressing more than one-third of cases (tickets) so far. The functional community continues to discuss how to better measure and communicate findings with the field, and checks in with end users for feedback once the issues are resolved, he assured.

“Feedback and tickets are helping us improve the system and improve the usability,” said Payne. “It’s really focused on the end users and how they use the system, so it’s great information for us. It’s a roadmap for us to get better.” It also has the potential to be helpful for the Department of Veterans Affairs as it moves forward with its EHR modernization effort utilizing the same record as DoD.

Payne said MHS GENESIS breaks down barriers to care, not only between the services and throughout the stages of a patient’s life, but also between Departments, fostering a collaborative culture that supports an integrated system of health and readiness.

Payne praised the testers at IOC sites for their great work during the initial phase of deployment, thanking them for making MHS GENESIS even better.

“We want to do as much as we can possibly do prior to wave one to make it go much smoother,” said Payne. “It’s kind of a cycle of continuous process improvement. We’re learning and we’re going to learn things from wave one that we didn’t learn from the IOC sites. We are improving the system and I think we’re going to get some momentum as we move forward. Our goal is to make wave one wildly successful. We want it to be a home run.”

Les émissions des États-Unis ont quelque peu diminué depuis 2005 en raison de changements technologiques, tels que la diminution de la production d'électricité au charbon face à la montée en puissance du gaz naturel et la croissance des énergies renouvelables.
